The HWM Client Service Center is designed as a convenience for clients that wish to initiate an on-line request for service. Please click onto the appropriate section below, complete as much information as possible, then click SUBMIT.
Please note that upon the receipt of a service request, our office will send you a confirmation and also inform you whether or not we are able to immediately process your request, or as the case may be, that we need to prepare a needed form as required by the insurance or investment provider, for your signature

Contact Change Form - To inform us of a change in your contact information (such as a new physical or email address or new phone number) or to update bank account information for those making automatic insurance payments or investment contributions, or receiving systematic investment distributions directly to your bank account.
Distribution Request Form - To request a one-time withdrawal request from an investment account(s).
Systematic Distribution/Contribution Form - To establish a systematic (on-going) distribution from an investment account(s).
Miscellaneous Request - To inform us of any other requests.
