It will help us set the baseline for developing a successful game plan to define and achieve your goals. Our clients also find this tool useful in taking stock of their beliefs and situations that they might not normally think about.
Please complete as much of this document as you can and we will use it in our initial meeting. If you think any of the pages are not applicable or unnecessary, just skip them. Some people find it easier to simply attach documents such as recent investment account activity and tax returns instead of filling in fields. That works just as well.
